HOW TO FILL OUT THE SUPPORT FORM ONLINE?

  1. Log in to your EDS system;
  2. Click on “Fill in the form” > “Request for support” > “Form code” FR0512. Select this form for electronic filling and click on the globe image – “Fill the form directly on the portal”;
  3. Fill in your personal information in fields 1, 2, 3v, 3p, 4;
  4. In field 5, enter the year of the tax period – 2020;
  5. Mark box 6S on the form;
  6. In line E1, select field 2 – “Beneficiary”;
  7. In field E2 of the line, enter the beneficiary’s code – 303267538;
  8. In field E3 of the line, enter the name of the beneficiary – A. GUOGOS PARAMOS FONDAS;
  9. In field E4 of the line, indicate the share of income tax – 1.2%.
  10. If you want to give support to several recipients, you can split the percentages. The total share of income tax is 1.2%;
  11. In field E5 of the line, indicate the year in which you want to allocate support. Until 2024 at the latest;
  12. Click Submit.

HOW TO FILL OUT THE SUPPORT FORM?
